To sign up for a class, please print and fill out our Application below and mail with applicable fees to: The Companion Dog Training Club, c/o Sandra Meade, 1 Linden Drive, Newburgh, NY 12550. ***Driving Instructions: Top Hat Dog Training, 108 Windsor Highway, New Windsor, NY 12553 / DIRECTIONS: I-84 E to Exit 7S, stay on Rt 300 S to * T-intersection (approx. 2 miles), turn left, staying on Rt. 300 to light at Union Avenue (second light). Turn left at Union Avenue. Go approx. 1.5 miles to light at Rt. 32. Turn left at Rt. 32 and go approx 0.5 miles to Top Hat Dog Training/Reis Group Insurance on left (Devitts on right). Turn left into driveway and go up hill to parking area. / I-84 W to Exit 7S and follow as above; / NYS Thruway (I-87) Exit 17; stay straight after toll booth to Rt. 17K. Turn right at 17K and then left at next intersection. Take Rt 300 S & follow as above

Your training fee entitles you to class membership in the Companion Dog Training Club of Newburgh, Inc. during the training sessions. I (we) agree to abide by the by-laws, rules and decisions of the Club and do hereby release the Club and all persons connected with it in any capacity whatsoever from any and all liability, cost and expense resulting from an injury to my (our) person(s) or property, or to the dog brought by me (us) to a training class or other event sponsored by this Club.
**If at any time it is felt that the behavior of a dog and/or handler jeopardizes the safety and/or well being of our clients, their dogs and/or our club members, we reserve the right to discontinue services to that handler and dog.**
